The Robert Wood Johnson Community Navigation Program is dedicated to helping individuals and families access the healthcare services and resources they need. The program provides personalized guidance to ensure patients can easily navigate high-quality inpatient and outpatient care, preventive health services, community education programs, and health screenings.Recognizing that effective communication is essential to quality healthcare, the program works to eliminate language barriers and support patients throughout their healthcare journey.
Community Navigators can assist with:
-Finding healthcare providers and scheduling appointments
-Coordinating transportation when needed
-Navigating referrals and specialty care services
-Facilitating diagnostic testing and specialist visits
-Advocating for patients and communicating with healthcare providers
- Developing community education and outreach initiatives
